The defense ministry on Monday released a plan for preventing accidents. That’s after two jets collided last week, leaving one dead and one missing.

It was the fifth major accident for the air force in just two years. Those crashes left a total of 12 dead and two missing.

Vice Defense Minister Chang Che-ping spoke at the Legislature on Monday about how the ministry is working to prevent air crashes. He says they’ve identified four areas for improvement, including: organizational culture, supervisory protocols, flight instruction, and hardware maintenance.

The defense ministry says the air force will place greater emphasis on training, supervision, certification and safety. That’s with the Aviation Safety Council saying that nearly two-thirds (65.9%) of all air accidents are caused by human error.

Chang also says there are problems within the organizational culture of the air force. He says rookie service members are often unwilling to bring up safety issues that they experience during training. He also says that air force pilots may be so focused on salvaging their aircraft that they miss the window of opportunity to safely eject from the jet.
